What is Sur Name, Middle Name,?

When you book your air ticket, they ask for this question, Normally South Indian will not have any Sur Name. Middle Name (what Moms name). Such question Irritate the Penger. they can very well give a big line. and North Indians can write their name in a Paragraph. South Indians cannot follows North Indian Styles, Please answer
